1. Privacy Policy

We take our client’s privacy very seriously. This policy explains how we collect and use personal and health information.
 

All persons, staff, contractors, volunteers and students who come into contact with, or have access to, confidential information have a responsibility to maintain the privacy, confidentiality and security of that information. All staff members, contractors, volunteers and students will be required to sign our privacy and confidentiality form.


First Peoples’ Health and Wellbeing will always look after personal information in a way that protects privacy and adheres to the National Privacy Principles and the Victorian Health Records Act. Clients may request a copy of our privacy policy at any time.

2. Why and when your consent is necessary

When someone signs up as a new patient at First Peoples’ Health and Wellbeing, they provide consent for our team to access personal information so that we can provide the best possible healthcare. If we use their information for anything else, we need to get their consent first.


If they do not consent to provide us with the required personal information, we may be unable to provide services to them and/or their diagnosis or treatment may be inaccurate or incomplete.

3. What First Peoples’ Health and Wellbeing staff are committed to:

  • You will only access information you need to do your job.

  • You will not disclose, copy, release, sell, alter or destroy any confidential information, either electronic or paper based unless it is part of your job. If it is part of your job to do any of these tasks, you will follow the correct procedure

  • You will not misuse or be careless with confidential information.

  • You will not disclose personal computer passwords and will only use shared passwords in authorised situations.

  • You accept responsibility for all activities undertaken using your password.

  • You know that your access to confidential information may be audited.

  • You will report any activities to my manager that you suspect may compromise the confidentiality and integrity of information. You understand these reports, made in good faith, will be held in confidence to the extent permitted by law.

  • You will protect the privacy of First Peoples’ Health and Wellbeing patients and employees.

  • You will be responsible for use or misuse of confidential information.

  • You understand your obligations under this Policy will continue after termination of your employment.

 

Any breaches of the above will be considered misconduct and may result in instant dismissal, and could result in prosecution under Victorian and Australian law.

4. Medical records

We take steps to ensure all medical records are:
 

  • Up to date

  • Accurate, complete, well-organised and legible

  • Contain enough information to allow another health provider to care for the client

  • Contain a summary of care

  • Can be used to remind clients about your follow-ups, check-ups and reviews

 

We collect, hold and use information for the following purposes

  • to provide medical services and treatment, and to enable clients to be attended by medical practitioners or other health practitioners

  • for administrative and billing purposes;

  • be used to assist in the running of First Peoples’ Health and Wellbeing

  • be shared when required with relevant individuals involved in the client’s healthcare

  • be used in a way that does not personally identify any client in reports for organisations that provide funding to First Peoples’ Health and Wellbeing

  • contact clients about my health and remind you about your appointments

  • comply with any laws or regulation requirements e.g. notifiable diseases

  • be added to national and state reminder systems including cancer screening and immunisations


If clients wish to remain anonymous when using First Peoples’ Health and Wellbeing, they must talk to the clinic team.

5. Complaints

If you receive a complaint our query about privacy concerns, you must direct the client to put their query in writing

The site manager will be required to acknowledge and respond to all complaints about privacy within 30 days.
